COVID-19 Wellness Check

UNC-Chapel Hill undergraduate students, graduate students, post-doctoral fellows, faculty and staff can be vaccinated for COVID-19 through Campus Health.

There is no out-of-pocket cost for this vaccine, but please bring your insurance card. Your insurance will cover administrative costs.

Vaccines are available at the Student Stores Pharmacy on the top floor of UNC Student Stores as well as the Campus Health Pharmacy in the basement of Campus Health. No appointments are necessary; visit either pharmacy between 9 a.m. and 5 p.m., Monday through Friday.

Off-Campus Vaccination Sites

In addition to Campus Health or the Student Stores Pharmacy, you may find vaccine locations listed on the North Carolina Department of Health and Human Services website.

Once you have received your vaccine, please complete the COVID-19 Vaccination Certification form.
Enter the date(s) and type of vaccine you received, and upload proof of your vaccination status.
The employee should schedule or decline the Hepatitis B vaccine within 10 working days after the exposure determination is made for an employee (see the Exposure Control Plan for your work area on the Bloodborne Pathogens page). The vaccine is offered by UNC-Chapel Hill at no cost to the employee.

Student workers need to contact UNC Campus Health Services.

University employees who wish to obtain the Hepatitis B vaccination need to complete these steps:

  1. Fill out the Hepatitis B vaccination consent form.
  2. If you don’t have a medical record number, call 984-974-8150 to get one.
  3. Schedule your appointment with the University Employee Occupational Health Clinic at 919-966-9119.

To decline the Hepatitis B vaccination:

  1. Complete the Hepatitis B vaccination declination form
  2. Follow the directions on the form to submit it to the University Employee Occupational Health Clinic.

Health Care Personnel (HCP) are defined as all paid and unpaid persons working in health care settings who have the potential for exposure to patients and/or to infectious materials, including body substances, contaminated medical supplies and equipment, contaminated environmental surfaces or contaminated air. Health Care Personnel who work in clinical environments, including UNC School of Medicine and UNC Hospitals clinics, Campus Health Services and UNC School of Dentistry are covered under this requirement. More information about vaccinations for health care personnel can be found at the CDC’s website.

Note: quantitative anti-HBs titer are required for any health care provider identified as performing Category 1 exposure-prone procedures
Influenza Annual influenza vaccine unless medical or religious contraindication
MMR Birth before 1957 (unless in outbreak situation) OR
2 doses of MMR given at least 4 weeks apart OR
2 doses of MMR (initial plus booster at anytime afterward) OR
laboratory confirmation of disease OR serologic immunity
Note: HCP with documented doses of MMR with negative or equivocal titers are considered to be immune to measles, mumps, and/or rubella
Varicella 2 doses of varicella vaccine given at least 28 days apart OR
1 dose of herpes zoster vaccine AND age >50 years OR
Treating physician diagnosis of varicella or herpes zoster OR
laboratory confirmation of disease OR serologic immunity
Tetanus, diptheria, pertussis (Tdap) One dose of Tdap within the past 10 years (provide HCP has received a primary series of Td immunizations). Pregnant HCPs need repeat doses during each pregnancy. Booster Td/Tdap every 10 years.
Meningococcal One dose to microbiologists/staff who are routinely exposed to isolates of N. meningitides. Booster every 5 years.
Other
Tuberculosis Surveillance Program Documentation of two-step Mantoux TST at any time point (e.g. two TSTs placed with a 12 month time interval) AND a negative TST or IGRA test within the past 12 months prior to hire. While a single IGRA test is sufficient to test for latent infection with M. tuberculosis, UNC-Chapel Hill and UNC Health do not use IGRA testing for outbreak investigations; therefore a documented two-step TST is required. Annual TST and symptom review are recommended but not required.
Note: HCP with a history of BCG vaccine and/or history of treatment for active or latent TB have different TB surveillance requirements. See TB protocol.

Please note that the above information is targeted to health care personnel working in UNC-Chapel Hill and UNC Health patient care locations. Separate protocols continue to be in place for research, DCM, long-term care facilities and other programs. Please talk with your supervisor if you are unsure what immunization and TB surveillance protocol is required for your specific work location.

University policy requires University employees traveling internationally on University-sponsored business to be seen at the University Employee Occupational Health Clinic (UEOHC) prior to their trip. Appointments should be scheduled as soon as the employee becomes aware of their travel plans, preferably six to eight weeks before departing the country. Employees receive a pre-travel medical evaluation and education about prevention of illness, prescriptions for prophylaxis against diarrheal and vector-borne disease, and administration of necessary immunizations. Available immunizations include those for yellow fever, Japanese encephalitis, typhoid, hepatitis A, rabies, meningococcal meningitis, tetanus, polio, diphtheria and pertussis. If you become ill during or after your trip, the UEOHC will also provide post-travel assessment for diagnosis, treatment, and necessary follow-up.
